Parker Landing will enroll any child regardless of race, creed or color.

The following forms need to be on file in order for registration to be complete:

1. Enrollment Application
2. Medical Form
3. Authorization Form

The medical form must be on file in our office and signed by your physician, physician assistant, or nurse practitioner upon enrollment.
