Parker Landing will enroll any child regardless of race, creed or color.
The following forms need to be on file in order for registration to be complete:
1. Enrollment Application
2. Medical Form
3. Authorization Form
4. Emergency Cards
The medical form must be on file in our office and signed by your physician, physician assistant, or nurse practitioner within 30 days of enrollment |
